Patient Services Registration

Register for Online Patient Services to be able to order repeat prescriptions and make or cancel appointments online.

You can sign up to Online Patient Services by completing the application form and returning it to the practice in person, along with a valid form of photographic identification, for example photo ID or your passport. Forms are also available from Reception.

Once you are registered the practice will give you the information that will enable you to create a username and password.

• view when you last ordered your medications
• make and cancel appointments online
